St.Vincent Privacy and Confidentiality Statement
Under NO circumstance will St. Vincent voluntarily disclose your personal information to your employer, or any other party without your specific consent or as required by law.
Use and Disclosure Policies
The following paragraphs disclose St. Vincent's information gathering and dissemination practices for this Web site.
In order to provide you with accurate, useful and customized information, St. Vincent asks for the minimally necessary data which may include your name, address, email and relevant health history along with information about lifestyle behaviors such as smoking and exercise, physical measurements such as height weight and blood pressure, and mental health topics. Your answers form the basis of the personal information that is unique to you. In conjunction with the customized information provided, you may also receive follow-up mailings.
Any transactions performed requiring your credit card information are performed in a secure environment, and credit card information is discarded immediately upon completing the necessary transaction(s).
Within the web site, you may be given the opportunity to indicate your preference to receive program information or participate in future research studies. If you indicate a preference to receive information, St. Vincent will use your contact information to send you information about St. Vincent, material about related products, or invitations to participate in research studies to assess the effectiveness of St. Vincent's programs.
Regardless of your response, you may opt-out of receiving future mailings by following the steps in the Choice/Opt-Out section below.
St. Vincent will create aggregate data results for dissemination to sponsoring organizations and for research purposes. THIS AGGREGATE DATA WILL NOT CONTAIN ANY INFORMATION WHICH DISCLOSES YOUR IDENTITY. In this case, your data is combined with data of other participants to determine an overall profile of a population. These data become statistics and may be used by St. Vincent, your employer, health plan, or other organizations to understand the needs and characteristics for selected populations.
St. Vincent uses your IP address only in the event a Web server problem requires diagnosis and to administer the Web site and to count aggregate unique visits to the web site.

Authorized Access: You must be registered to use portions of this Web site. Once you are registered, you will use a unique user ID and password to enter the site.
Authentication: St. Vincent uses Verisign to provide authentication, trust, and SSL encryption. This allows site visitors to verify St. Vincent with a trusted third party organization, while enabling communication with St. Vincent's web sites using secure, encrypted channels.
Cookies: A cookie is a piece of data containing information about the user or user session. Usage of a cookie is not in any way linked to personally identifiable information while on the St. Vincent site. St. Vincent uses a session cookie to ensure that your session remains secure by maintaining an identifier unique to your session. The session cookie self-terminates after you close your browser. Persistent cookies are only used if you choose to have your login and password information maintained for future connections. The default setting is set to not store login and password information.
Encryption: St. Vincent employs SSL encryption in the use of its products. We recommend the use of a browser supporting 128-bit encryption though the site will support down to 40-bit encryption.
St. Vincent is committed to protecting the privacy of children. ST. VINCENT'S PRODUCTS ARE NOT INTENDED FOR USE BY ANYONE UNDER THE AGE OF 18. In the event the system recognizes that anyone under 18 has attempted to send personal information through questionnaires, the session will be aborted and an appropriate notice posted to the user.

Notice and Procedure for Making Claims of Copyright Infringement
- A physical or electronic signature of a person authorized to act on behalf of the owner of the exclusive right that is allegedly infringed;
