Now you don’t have to have a physician to receive the care you need. Health Connect provides immunizations, physicals and screenings – all that you need to help prevent most diseases or conditions. Health Connect: Here's What to Expect

• You’ll complete short questionnaires to help assess your health and identify your risk factors.
• Together, we’ll review your healthquestionnaires, past medical history, family medical history and social history.
• A basic physical exam will focus on your specific risk factors, along with needed lab work and immunizations.
• We’ll help coordinate any follow-up care, tests or procedures you may need.

Health Connect: Passport to Travel

• Our travel consult is for individuals or groups who plan to travel internationally.
• We’ll ask you several questions to help determine your health risks and medical needs.
• Any necessary vaccines are administered and prescriptions written.
• You’ll get a vaccine certificate for your records, along with care information you may need before you travel.
• We’ll provide you with our contact information so you can reach us during your travel if you have questions.
• We’ll provide immunizations for most preventable diseases, including yellow fever, Japanese encephalitis, typhoid, Hepatitis A, Rabies, meningococcal, and others.

Test results are generally available within 24 hours and can be delivered via mail, telephone, by e-mail or kept at the office for pick-up.
