Discharge Planning

St.Vincent Medical Center Northeast has well-trained and experienced health care professionals to answer questions, assist you and your family and help ease the transition to your home and community. St.Vincent staff will help arrange for the services you need such as home nursing care, therapy, housekeeping services and assistance with medications.

To make your discharge as smooth as possible, your physician will notify you of your discharge at least 24 hours before you are to leave the hospital. If you have questions related to leaving the hospital, please consult your nurse.

Before you leave your room, please be sure to check through all of the closets and drawers so you do not leave behind any personal items.
