Health Access   

Health Access is a program in which case managers can connect those who are uninsured to a primary care physician and other services.

Why do you need a primary care physician? A primary care physician will get to know your needs and the needs of your children—which means better health care.

Health Access Coordinator

The Health Access Coordinator can also connect you to:

•  Project Access
•  Transportation to medical appointments
•  Medications
•  Healthy Indiana Plan (HIP)
•  Mental health Services
•  Emergency food
