CardiAction

CardiAction is a multi-disciplinary approach to the identification, treatment, and management of coronary artery disease and congestive heart failure. It is nurse managed and physician directed. The program's objective is to take "Action" against "Cardiac" disease. CardiAction acts as an "umbrella" over three specific areas: the Cardiovascular Risk Factor Management Clinic (smoking cessation, lipid treatment, cardiac rehabilitation), Endocrinology and the Congestive Heart Failure (CHF) Clinic.

Medical errors are a leading cause of death. Recent data from the Centers for Disease Control and Prevention (CDC) show that emergency room visits have hit a record high of nearly 114 million visits per year. Providing patients with access to their medical records at the point of care will save lives and produce better outcomes. Policy makers from both political parties are pushing for healthcare reform.

We are taking an active approach to reduce medical errors for our patients. While the CDs are available through other cardiology practices in the United States, our joint effort with St.Vincent Heart Center of Indiana is the first to provide cardiovascular information from both a cardiology practice and hospital.

Order MyRecords

We are offering piece of mind to our cardiovascular patients. The MyRecords credit card-size compact disk contains your most current cardiovascular medical information and history pulled from our electronic medical record and includes cardiovascular records received from St.Vincent Heart Center of Indiana.

For more information or to order your CD, please complete the order form and mail a $25.00 check payable to St.Vincent Medical Group. You may wish to order additional CDs for family members and should consider requesting an updated CD on an annual basis or if there is a major change in your cardiovascular health throughout the year.

The CD also contains education about their diagnosis, treatment and medications. Just like any other private data, it is the patients responsibility to keep the CD secure.

Community Education

Our community education offers:

  • Cooking for a Lifetime: This cookbook contains over 300 heart healthy recipes designed to make low fat cooking easy & delicious. It also contains the American Heart Association Guidelines and tips for modifying recipes. Many of the recipes were developed by chefs from around Indiana. $15
  • The Guide to Heart Healthy Living: This comprehensive guidebook contains information on risk factors for heart disease, and information on how to lower your risks through lifestyle changes. Weight management, exercise, and stress reduction topics are also covered. Additionally, it contains sample menus, information on increasing fiber in your diet; and a summary of possible interactions between herbal products & prescription medications. $15
