Community Outreach
The Community Outreach Department provides three services to support families with special needs:




Family and Community Services (FCS)
Through one-to-one mentoring relationships, children and young adults gain critical life skills to live more independently. Family and Community Services provides support services within a child’s or young adult’s home and community, including:
- Independent living skills
- Making connections with community organizations
- Follow through on therapy and educational goals
- Access to behavior consultants and nurses
- Housing and related support
- Personal assistance
- Accessible transportation
- Counseling and behavior management
- Nursing/health care Coordination
Funding Sources
Funding sources include:
- Medicaid waiver
- Private fees
Home-Based Counseling Program
This program provides intensive home and community-based services to families referred through the Department of Child Services or Juvenile Court. We provide teaching and support in the areas of:
- Child development
- Nutrition
- Housekeeping
- Cooking
- Positive discipline
- Recreation
- Parenting instruction and support in accessing other programs which may benefit the family
Funding is provided through the Department of Child Services.
Day Services
St. Vincent New Hope Day Services is an activity-based program serving adults who have intellectual and physical disabilities. Our mission is to provide meaningful days to the individuals we serve by developing their personal areas of interest. We offer individuals the opportunity to shape their own day through personal choice and preference, while still participating in a structured day. We also work to connect those we serve with their own communities.
