Supported Living Programs
This program provides guidance, resources and support services to help people with developmental disabilities reach their highest potential and to live in the community.
Two to four individuals live in their own home or apartment. Services are provided up to 24 hours a day, 7 days a week at these residences in pleasant, secure environments. Most homes owned by St. Vincent New Hope are wheelchair accessible.
Services
Support, services and opportunities include:
- Training in money management, self-care, community access, household skills
- Healthcare coordination
- Behavioral and counseling support
- Transportation for vocational programs, appointments and community involvement
- Person-centered planning to identify hopes, desires and needs
- Wheelchair and adaptive equipment support and maintenance
- Physical therapy consultation
Funding
Sources of funding include:
- Medicaid waiver
- Developmental disabilities
- Autism
- Traumatic brain injury
- Aged and disabled
- Private fees
