Care Manager Program

The Care Manager Program at St. Vincent is dedicated to providing support to our older clients and their families when they do not know where to go for necessary services. Our licensed healthcare professionals are dedicated to providing spiritually-centered holistic care that sustains and improves the health and well-being of individuals.

This fee-for-service program is designed to evaluate for specific needs, find solutions and arrange for services to maintain independence at home.
