INDIANAPOLIS – St.Vincent Home Health Care is celebrating its recent recognitions by two prestigious national organizations recognizing their quality outcomes and exemplary financial performance.
The faith-based home care program was honored by OCS, Inc., the leading post-acute healthcare information company, and named to its HomeCare Elite status. HomeCare Elite awards the most successful Medicare-certified home health care providers in the United States, and St. Vincent was ranked among the top 25 percent nationally.
Also, Health Care Excel, the Indiana Medicare Quality Improvement Organization (QIO), honored St. Vincent Home Care for its national Home Health Quality Improvement Award of Merit. St.Vincent participated in the Medicare QIO to improve its performance in specific outcome measures such as reduction in avoidable acute care hospitalization.

Although based in Indianapolis, St.Vincent Home Health Care also provides compassionate care for patients in approximately 25 counties, and has rural offices in Jennings and Fountain counties. The multidisciplinary team of nurses, therapists and home health aides provide care to approximately 4,000 patients annually ranging from the geriatric, joint replacement, cardiovascular and oncology specialties.
St.Vincent Home Health Care routinely receives high patient satisfaction scores as it remains focused on helping patients recover in the comfort of their own homes.
The comprehensive program recently invested in telemonitoring technology to monitor heart failure, pulmonary disease and diabetic patients at home, and prevent the chance of re-hospitalization. The technology is installed in a patient’s home, and their medical information is transmitted to a secure website. A home care nurse conveniently communicates with and monitors the patient’s vital signs on the website from a workstation in the office. St.Vincent Home Health Care also visits the patient for follow-up care.
