ANDERSON – On Tuesday, June 2, St.Vincent Health, Saint John’s Health System in Anderson, and the Community Health Access Project (CHAP) will introduce the Pathways Care Coordination Model to Indiana health and state officials. The Pathways Care Coordination Model identifies at-risk individuals; ensures they connect with evidence-based intervention; and measures the health outcome and cost savings. It is designed to address high priority community problems identified through evaluation of data and community input. In Indiana, there is no broad-based strategy to identify those most at risk, or measurement of these patients’ health outcomes.
Populations at risk for poor outcomes in pregnancy, diabetes, hypertension and depression often enter emergency rooms to seek primary care. Doing so proves to be detrimental and expensive for patients and the current national health system. Based on the Pathways model, connecting at-risk individuals to health care, behavioral health and social service interventions at an early stage prevents poor outcomes, reduces expenses and improves quality of care.
Indiana leaders will gather at Saint John’s Medical Center to learn about CHAP’s model of community-based care coordination, which aims to improve the basic health and social outcomes of individuals and neighborhoods. Drs. Mark and Sarah Redding of CHAP will share examples of their collaborative efforts with other states, and how these initiatives improved outcomes and saved money.
Madison County was identified as the meeting location due to its growing uninsured and underinsured population, as well as high community rates of diabetes, blood pressure, cholesterol, depression, teen pregnancy and smoking.
