Application Information
The resident must be a pharmacist having sufficiently broad knowledge, skills, attitudes, and abilities in pharmacy practice necessary for further professional development at an advanced level.

 

Applicants must possess a Doctor of Pharmacy degree from a college accredited by the American Council on Pharmaceutical Education, and must hold or be eligible for pharmacist licensure in Indiana.  Applicants must have completed an ASHP-Accredited PGY1 pharmacy practice residency.

 

A completed application consists of the following:

  • Letter of intent specifying career objectives in primary care
  • Curriculum vitae
  • College transcripts
  • Three letters of reference,  one of which must be from current program director
  • Onsite interview presentation
  • St.Vincent Hospital Employee application prior to start of the residency available at www.stvincent.org. Then select:
    • Find a career
    • External (New applicants)

 

Screening interviews will be conducted at the ASHP Midyear Clinical Meeting.    Formal interviews for qualified candidates will be conducted in Indianapolis. All interview candidates will be required to present a minimum of a 30 minute PowerPoint presentation on a topic pertinent to primary care. The audience will be the pharmacy and medical staffs.  Copies of the presentation will be due three days prior to the interview.

 

All application material must be received by January 15th.

Residency Program

The ambulatory care pharmacy resident will actively participate in rotational, perennial and annual learning experiences.  The residency preceptors will provide written learning objectives for each learning experience. These objectives specify the skills and knowledge the resident will gain upon successful completion of each experience.  The resident will be provided a training plan to help guide performance and expectations, track achievements, and progress in obtaining individualized, as well as program goals. These training plans will be updated quarterly and will coincide with quarterly resident evaluations.  In addition, the resident will be expected to evaluate themselves, preceptors, and the strengths and weaknesses of each aspect of the residency program.

 

  • Longitudinal experiences:
    • Internal Medicine- minimum of 3 months
    • Family Medicine- minimum of 3 months
    • Pediatrics (elective)
    • OB/GYN (elective)
    • Institute on Aging
    • Asthma clinic
    • Anticoagulation clinic
    • Diabetes education clinic

Longitudinal Learning Experiences:

  • Family Medicine and Internal Medicine
    • The resident will actively participate in management of assigned physicians patients for a minimum of 3 months for each clinic during the year.
  • Anticoagulation clinic
    • The resident will be responsible for all aspects of anticoagulation care as assigned by preceptors.  Additionally, the resident, when assigned to Internal Medicine or Family Medicine clinics respectively will be responsible for management of all new anticoagulation patients
    • The resident will participate in ongoing outcomes-based research regarding anticoagulation management
  • Institute on Aging
    • The resident will participate in geriatric assessments one-half day a week for a minimum of 6 months.  Resident will be responsible for performing a medication history on each patient, review appropriateness of medications, recommend level of supervision with medications, provide adherence tools, and communicate finding and recommendations to multidisciplinary team.
  • Asthma Clinic
    • The resident will participate in asthma clinic with the respiratory therapist and physician to gain knowledge and skills necessary to provide comprehensive asthma education to patients and care givers.  Residents will also gain experience in performing and interpreting pulmonary function tests.
  • Diabetes Education Clinic
    • The resident will be responsible for providing and assessing diabetes education to patients as assigned by the preceptor.
    • The resident will document all activities and provide recommendations to the primary care provider and/or staff physician if changes in pharmacotherapy are warranted.
    • The resident will participate in ongoing outcomes-based research regarding diabetes management
  • Practice management
    • The resident will be involved in administrative processes as outlined above and will be further defined in the goals and objectives of the practice management experience.  The resident will be given dedicated time to fulfill practice management expectations throughout the residency.
  • Drug Information/Medication Safety
    • The resident will provide formal drug information in the form of newsletters, bulletins, and inservices to pharmacists, nurses and physicians in addition to extensive informal opportunities. 
    • The resident will have the opportunity to submit for publication a poster to present at a national meeting and/or an article for an appropriate journal. 
    • The resident will actively participate in the activities of the hospital Pharmacy and Therapeutic committee, the Primary Care Center Operations committee, as well as ad hoc participation with other relevant groups.
  • Teaching
    • For the first six months, the resident will participate in the teaching certificate program offered through Eli Lilly and Purdue University, if not already completed.
    • The resident will be required to give a minimum of 3 lectures for the Family Medicine physicians, and a minimum of 2 lectures to the Primary Care Center nursing staff
    • The resident will be required to give one ACPE-accredited lecture to the pharmacy staff, and will also be responsible for leading a case-based discussion for the pharmacy staff on the topic corresponding to the ACPE lecture.
    • The resident will be required to precept one student during the second 6 months of the residency experience
  • Residency Project
    • The resident will be expected to complete a residency project to be presented at the Great Lakes Pharmacy Resident Conference, St. Vincent Research Day, and that is suitable for publication.  The project must be approved by the Primary Care Center Operations committee, and the St. Vincent Investigational Review Board.  The project will be tailored around the interests of the resident as well as the needs of the organization.
  • Staffing
    • The resident will be required to staff weekends every three weeks. Responsibilities will be dedicated to the Orthopedic Anticoagulation Service and will include both operational and clinical functions.
