Any child, from newborn to teen, may be seen at the clinic. Pregnant teens or complex gynecological problems are referred to the OB/GYN clinic. Insurance accepted: Medicaid, Hoosier Healthwise and commercial insurance. The children without any insurance coverage are seen on a sliding fee scale (see below).
How to make an appointment
Call 338-7510 and follow the voice prompt for scheduling. An exception to this is the established clinic patient with an illness that needs to be seen that day. In these instances the parent needs to call the clinic nurse directly at 338-7532; it is important to make this call by 9:30 if possible as some days the clinic only meets in the morning. Walk-in appointments are not available. You should call your insurance carrier prior to making an appointment to verify that the Primary Care Center is an approved provider. Then the clinic doctor should be listed as their Primary Care Provider. If your child has a life-threatening emergency, you should go to the nearest emergency room by ambulance. Urgent nighttime problems can be addressed by the resident on call for pediatrics at 631-3466.
What should you bring?
For a new patient appointment, patients should bring immunization records, previous medical records, allergies and any medications they need to take (names and doses). If the child is not brought in by the legal guardian, written consent to treat must accompany the child (standard forms to allow someone other than the parent to bring the child are available at the clinic). Also bring a Medical card or insurance card. Those without insurance need to bring verification of income (see below) to help financial services determine the appropriate sliding scale charges.
What services are available?
Routine well-child care, immunizations, urgent sick care and newborn follow-up are provided. This includes assessment of growth and development, nutrition, sleep patterns, behavior problems and any health concerns.
The clinic is open Monday and Tuesday mornings from 9 a.m. -12 noon and Wednesday through Friday from 1 p.m. -4 p.m. The Supershots program provides free immunization clinics every month. Outreach education is also provided to teen moms at Washington Township schools.
Billing procedures
Families with private insurance are responsible for their co-pays at the time of the visit; insurance will be filed for covered services. Medicaid and Hoosier Healthwise coverage will be filed. Self-pay uninsured patients will be charged based on the sliding scale rate established by Financial Services at the time of their first visit. This rate is determined by family income, household members and other factors. Patients are asked to provide documentation of their financial status including three recent pay stubs, three current bank statements and current year tax forms. The discounted fee is expected at the time of service. |
