The Care Manager Program at St. Vincent is dedicated to providing support to our older clients and their families when they do not know where to go for necessary services. Susan DeWitt, RN, is a geriatric nurse with over 30 years experience in discharge planning and care management for seniors. This fee-for-service program is designed to evaluate for specific needs, find solutions and arrange for services to maintain independence at home.
