In order to recommend an appropriate course of treatment, we will want to know about you and the reason for your appointment. The more we know about you, your family and loved ones, the better we can help. When you arrive at St. Vincent Stress Center, you will be greeted at the information desk by our reception associate. At that time, we will gather demographic information such as your name, address, phone number, referral source, emergency contact, etc. For your convenience, some of our patient history forms may be completed ahead of time by following the links at the left. Please bring them along with your photo id and health insurance card (if applicable) to your first appointment.
