This program provides guidance, resources and support services to enable individuals to reach their highest potential and to live in the community. Generally, two to four individuals live in a home or apartment. Homes owned by St. Vincent New Hope are wheelchair accessible. Individualized support services are delivered to persons at their residence.

• Physical Therapy Consultation
