This program provides guidance, resources and support services to enable individuals to reach their highest potential and to live in the community. Individuals in their program generally live in apartment settings or a home. Homes owned by St. Vincent New Hope are wheelchair accessible. Individualized support services are delivered to persons at their residence.

 

  • Two to four individuals live in their own home or apartment
  • Support Services are provided up to 24 hours, 7 days a week

 

Support, Services and Opportunities

  • Training includes money management, self-care, community access, household skills
  • Healthcare coordination
  • Behavioral and counseling support
  • Transportation for vocational programs, appointments and community involvement
  • Person-centered planning to identify hopes, desires and needs
  • Pleasant, safe, secure environments
  • Wheelchair and adaptive equipment support and maintenance
  • Physical Therapy Consultation
