If it seems useful, seek help from a non-family member to guide conversations that are difficult for you. The case managers at St.Vincent are registered nurses with significant experience in working with oncology patients, and serve as an advocate for individuals and families who require cancer care.

The case manager works with the patient and all caregivers to facilitate access to needed services. The case manager can assist you in exploring options for post-hospital care. Once those decisions have been made, the case manager can assist you in making those arrangements and aid you in making a smooth transition to another level of appropriate care. This person also will communicate with your insurance company as needed, and facilitate communications between the patient/family and the health care team. Finally, your case manager can link you with available community resources as well as refer you to appropriate in-hospital and outpatient services, including:

  • support groups
  • extended care facility
  • oxygen
  • financial
  • home health care
  • durable medical equipment

 

Case management offices are located within the oncology unit and can easily be accessed through the nurse's station.
