Season Flu Vaccine Information

Most St.Vincent Physician Network medical practices are currently giving the seasonal flu vaccines. Some of the medical practices are also offering the live, intranasal (mist) vaccine. This page is designed to help current St.Vincent Physician Network patients receive the appropriate seasonal flu vaccine.

 

If you are a current patient of a St.Vincent Physician Network medical practice, please call your medical practice to schedule a date and time to receive the appropriate seasonal flu vaccine. Some of the medical practices are offering seasonal flu vaccine clinics. Please listen to the message when you call your physician’s office to hear details about flu vaccine clinics.

 

Below are the necessary forms and information you should review and complete prior to your scheduled appointment time or arriving at a seasonal flu vaccine clinic.

 

NOTE: The H1N1 vaccines will not be available until after October 15. The St.Vincent Physician Network will follow the recommendations set forth by the Centers for Disease Control and Prevention on which patient groups should receive the H1N1 vaccine. Also, special content forms will be used when the H1N1 vaccine is available in the future. Please check back on this website for more information in the future on H1N1 vaccines.
