INSURANCE

 

As a courtesy to our patients, we will file your claim as long as you have provided your insurance card to us at the time of service. Payment is required at the time of service. Our contracts with insurance companies we are participating state that it is required to collect all co-payments at the time of service. For your convenience, we accept cash, personal checks, Visa, MasterCard and Discover. If you do not have your insurance card, we require that payment be made in full at the time of service, and we will provide you with a copy of the bill to send your claim to your insurance company for reimbursement. It is also recommended that you keep a copy of all receipts we give you in case you need them for tax purposes.

 

After we file the insurance claim, it is your responsibility to follow-up on the payment of your claim and contact your insurance company if any questions arise regarding payment. If you have not heard from your insurance company within 30 days from the date of service, please call your insurance company to check on the status of the claim.

 

If there are extenuating financial circumstances, please notify our office and we will be more than willing to discuss options and resolutions to the problem. We accept plans the following insurance carriers; however, we ask that you please contact your insurance carrier to verify that we are participating with your particular insurance plan:
