How do I apply for CME credit 1?

 

Visit the Medical Education Intranet Website to review  "The Handbook for CME Program Planners" and obtain an on-line CME form.

 

This form must be submitted at least 30 days prior to the activity being presented. (For major programs, you should apply for credit approximately 6 months prior to the program).

 

PLEASE NOTE THAT THE CME COMMITTEE HAS THE AUTHORITY TO DENY PROGRAMS THAT ARE NOT SUBMITTED WITHIN THIS TIME FRAME.

  • Incomplete applications will be returned with a request for further information. Under no circumstances should CME accreditation statements appear on printed course material without written approval from the CME Subcommittee.
  • Members of the CME Committee will review programs via an electronic format and approval memos will be sent within 7 business days of the initial request.
  • Following approval, you will receive instructions for brochure design, evaluation, and any other relevant CME details.
  • Upon conclusion of the program, the following items must be submitted to the Medical Education Department:

 

    • Summary of Evaluations
    • A list of conference participants
    • A copy of brochure or other printed materials
    • Finalized Budget
    • Copies of Planning Committee disclosures and faculty disclosure forms
    • Commercial support letters of agreement
