At St. Vincent Home Health, we are dedicated to providing spiritually centered, holistic care that sustains and improves the health of individuals.
Our services are delivered in the home or assisted living facility by licensed nurses; physical, occupational and speech therapists and therapy assistants; medical social workers; home health aides; and a nutritional consultant, all of whom are associates of St. Vincent.
Patients may be referred for any of these services by a physician, hospital discharge planner, family member or friend, an outside agency, or by the patient. All care is provided under a physician's direction. Our services are available to any person, regardless of age, sex, religion, and mental or physical disability. St. Vincent Home Health is fully licensed by the State of Indiana.
Under the supervision of a medical professional, the home health aide assists with daily living needs, help bath and dress the patient, change dressings for wound healing, check vitals such as pulse, respiration and blood pressure, and help move the patient as needed using home medical equipment.

St. Vincent Home Care provides a Palliative Approach to Treatment at Home®, called the PATH Program. This program offers a team approach to address the needs of the patient with a severe illness, who is homebound, and have a skilled medical need. The medical therapies and treatments intended to cure the condition can be continued while in the PATH program. Unlike hospice, these patients do not need to have to have a life expectancy of six months or less.
The PATH team includes nurses, chaplains, social workers, and physicians. We treat the whole person, including body, mind, and spirit. For the family, we provide support to cope with the illness and grief support as needed.
This program is personalized for each patient and manages symptoms. We start by helping each patient and family understand the cause of the pain. Then with physician supervision, we provide medicine, therapies and other medicines to help alleviate pain and other distressing symptoms.
The PATH social worker helps patients and families explore options and get resources to make life at home more comfortable. Stress complicates healing and PATH addresses these: living alone, financial strain, lack of transportation, securing caregivers for the elderly or disabled, balancing family issues, and creating advanced directives.
Sometimes a patient will choose to transfer from PATH to hospice services. The PATH team will assist to ensure a smooth transition.
If you have heart disease, diabetes, high blood pressure or a lung disease like COPD, you may need to make lifestyle changes to stay healthy. In addition to following your physician’s instructions about diet, physical activity, and medication, it’s also beneficial for your healthcare team to track your vital signs and symptoms on a regular basis. With telehealth monitoring, your home health team can do just that, while you stay at home.

By recording your vital signs, a daily report is automatically sent to your home health care nurse. Whenever there are changes in your condition, a nurse will call you and your physician. The program is available to you at no additional cost while you are enrolled in home health services.

Home care is medical care provided in a patient’s home. The type of care provided by skilled medical professionals can be broad, including skilled nursing and primary types of therapy (physical, occupational and speech). However, home health care can also include skilled, non-medical care such as medical social services and daily living assistance provided by highly qualified home health aides.
Medicare.gov describes home health care as “usually less expensive, more convenient, and just as effective as care given in a hospital or skilled nursing facility.”

If you have Medicare coverage, you may choose a home health care provider that Medicare has approved. The Medicare benefit covers 100 percent of home care services when certain qualifying conditions are met. St. Vincent Home Health Care Services are Medicare-certified.

Medicare requires that a patient be “homebound” and have a “skilled need” to qualify under Medicare for home health nursing and therapy services. The patient is considered “homebound” under Medicare if the patient cannot leave without “considerable and taxing effort.”
The patient cannot be disqualified from services if they leave home for a non-medical absence as long as the absence is infrequent and brief. Some examples include attending a religious service, a trip to the barber or the hair salon, a special family event, or a walk around the block. The patient must have a “skilled need” that requires the skills of a licensed nurse, speech therapist or physical therapist.
